Allergia da contatto ai componenti di una ?serie integrativa profumi? in pazienti con dermatite eczematosa

Introduzione: i profumi sono una frequente causa di dermatite allergica da contatto (DAC). Per la diagnosi di DAC con profumi si utilizza profumi mix (PM). Esso consente di identificare il 70-80% dei pazienti allergici ai profumi, ma gli 8 componenti che lo costituiscono non permettono di rilevare l?allergia alle centinaia di essenze profumate, naturali o sintetiche, presenti nei profumi o nei prodotti profumati. Obiettivo: scopo del seguente studio è valutare la frequenza delle reazioni allergiche ad alcune essenze profumate non presenti in PM, ma riportate in letteratura come responsabili di allergia da contatto, calcolando la loro concordanza con le reazioni a PM. Materiali e metodi: sono stati arruolati 292 soggetti affetti da dermatite eczematosa che riferivano, all?anamnesi, intolleranza ai profumi e/o comparsa di lesioni eczematose in sedi tipicamente colpite dalla DAC con profumi (collo, polso, volto). Tutti i pazienti sono stati sottoposti a patch test. Oltre agli apteni della serie standard SIDAPA (Società Italiana di Dermatologia Allergologica, Professionale e Ambientale) che include PM additivato con sorbitan sesquioleato e balsamo del Perù (BP), è stata testata in vaselina una ?Serie integrativa profumi? costituita da 14 essenze profumate. Risultati: dallo studio è emerso che, dei 292 soggetti, 195 hanno presentato positività a PM e/o BP , mentre 116, 24 dei quali negativi a PM e/o BP, sono risultati positivi ad una o più essenze profumate della ?Serie integrativa profumi?. L?essenza risultata più spesso positiva è olio di ylang- ylang; seguono lyral, olio di narciso assoluto, aldeide alfa-esil-cinnamica, gelsomino assoluto, farnesolo, citrale. Conclusioni: il nostro studio conferma che una mancata positività a PM non esclude una sensibilizzazione ai profumi. Da quanto riportato emerge l?importanza di testare, nei soggetti con intolleranza ai profumi e in aggiunta alle essenze profumate già contenute in PM, anche lyral, olio di ylang-ylang e narciso assoluto

Allergic contact dermatitis to the components of an ?additional fragrance series? in patients with an eczematous rash

Introduction: fragrances are a frequent cause of allergic contact dermatitis (ACD). Fragrance mix (FM) is used to screen fragrance allergy. It allows us to identify the 70-80% of patients allergic to fragrances, but the eight individual essences contained in this mixture are not able to identify allergies to the hundreds of fragrances, natural or synthetic, contained in perfumes or sweet-smelling products. Objective: the aim of this study is to evaluate the frequency of allergic reactions to some fragrances that are not included in FM, but that are reported in the literature as being responsible for ACD, by considering their concurrence with the reac- tions to FM. Materials and methods: 292 patients with eczematous dermatitis were studied. They showed intol- erance to fragrances and/or eczematous lesions on the neck, the wrist and the face, regions typically affected by ACD from fragrances. The patients were patch tested with the SIDAPA standard series that contains FM and balsam of Perù (BP), and with an ?integrative fragrances series? that contains 14 essences tested in petrolatum. Results: reactions to FM and/or BP occurred in 195 out of 292 subjects, while 116 patients, 24 of which were negative to FM and/or BP, showed a positive reaction to one or more essences of the ?integrative fragrances series?. The most frequent positive reactions were to ylang-ylang oil, followed by lyral, pure narcissus oil, alfa- hexyl-cinnamic aldehyde, pure jasmine, farnesol, and citral. Conclusions: our study confirms that a negative reaction to FM cannot exclude a sensitization to fragrances. It was shown that in the patients with an intoler- ance to fragrances, it is important to also test for ylang-ylang oil, lyral and pure narcissus oil in addition to the eight individual essences contained in FM.

Ciclosporina nell'orticaria cronica: stato dell?arte

L?uso di ciclosporina A (CsA) è ormai consolidato nella psoriasi e nella dermatite atopica, patologie in cui è approvato, ma ha un ruolo importante anche in altre malattie cutanee refrattarie ad altre terapie convenzionali come, ad esempio, l?orticaria cronica (OC). Infatti, numerosi studi in aperto hanno evidenziato l?efficacia e la tollerabilità di CsA in forme di OC di natura apparentemente idiopatica, caratterizzate da note- vole gravità dei sintomi e resistenza alla terapia antistaminica. La superiorità di CsA rispetto a placebo in queste forme di OC è stata anche confermata da due studi controllati randomizzati, che hanno valutato l?attività del farmaco sia nella vecchia formulazione sia in quella nuova in microemulsione. Numerose evidenze suggeriscono alcune delle possibili modalità d?azione di CsA nell?OC, legate non soltanto agli effetti immunomodulanti ma anche ad effetti intrinseci antinfiammatori, come quelli diretti nei confronti di mastociti e basofili.

Cyclosporin in chronic urticaria: state of the art.

The use of cyclosporin A (CsA) is well established in psoriasis and atopic dermatitis, for which it is currently approved, but it also has an important role in other diseases, such as chronic urticaria (CU), which are refractory to standard traditional treatments. In fact, several open-labelled studies have shown the efficacy and tolerability of CsA in cases of CU with an unknown etiology and characterized by severe symptoms which were poorly controlled by conventional treatment with systemic H1-receptor antagonists. The effectiveness of CsA over the placebo in these forms of CU has been confirmed by the results of two randomized controlled trials performed with either the old formulation of CsA, or the new oral microemulsion. The rational basis for the use of CsA in severe refractory CU is provided by a lot of evidences which suggest that the possible mode of action is linked not only to the immunomodulating activity but also to intrinsic anti-inflammatory effects, including those directly induced on mast cells and basophils.

Dermatite allergica da contatto delle palpebre da topici oftalmici. Il ruolo dei β-bloccanti: problemi di diagnosi e gestione

Introduzione: i β-bloccanti, farmaci antagonisti dei recettori β-adrenergici, sono largamente utilizzati nelle preparazioni topiche per il trattamento del glaucoma ad angolo aperto. Le dermatiti allergiche da contatto (DAC) da β-bloccanti contenuti nei colliri sono rare ma, per la scarsa sensibilità del patch test quando eseguito con le preparazioni farmaceutiche, possono risultare di difficile interpretazione. Materiali e metodi: in 4 pazienti affetti da glaucoma, che presentavano manifestazioni eczematose delle palpebre (in 2 casi era presente anche congiuntivite), sono stati praticati patch test con gli apteni della serie standard SIDAPA e di una serie integrativa ?Palpebre?, e con quelli della nostra serie ?Veicoli e conservanti?. Inoltre, abbiamo testato i componenti del collirio e una serie costituita da β-bloccanti maimutilizzati in precedenza dai pazienti. Risultati: in tutti i pazienti sono state registrate reazioni positive ai patch test con i β-bloccanti sospettati; inoltre in 2 di essi sono state osservate reazioni verso β-bloccanti mai utilizzati in precedenza. Conclusioni: la diagnosi e la gestione dei pazienti con DAC da topici oftalmici contenenti β-bloccanti può risultare complessa. Noi consigliamo di praticare i patch test nei siti classici (dorso o avambraccio) ma con concentrazioni più alte di quelle presenti nei prodotti oftalmici del commer- cio; in caso di negatività, è opportuno procedere con test epicutanei modificati su cute scarificata o ?stripped?, al fine di simulare l'assorbimento più alto della cute palpe-brale. Inoltre è opportuno testare i pazienti con una serie di β-bloccanti alternativi per scoprire eventuali reazioni crociate così come effettuare un test d'uso con il prodotto farmaceutico alternativo.

Allergic contact dermatitis of the eyelids from topical ophthalmic medications. The role of β-blockers: diagnostic and management problems.

Background: β-blockers, i.e. β-adrenergic receptors antagonists, are drugs which are widely used in local medications for the treatment of open-angle glaucoma. Allergic contact dermatitis (ACD) due to the β-blockers contained in eyedrops are rare, but it can be difficult for dermatologists to diagnose and manage because of the low sensitivity to patch tests with such drugs and of the possible cross- reactions between them. Materials and methods: in four glaucoma patients with signs of eczema on their eyelids (in two cases conjunctivitis was also present) patch tests were carried out using haptens of the SIDAPA stand- ard series, of a palpebral series and from our series of vehicles and preservatives. Moreover, we tested the eyedrops? components and another series containing β-blockers that the patients had never used before. Results: all patients reported positive reactions to patch tests containing the suspected β-blockers. Moreover, in two of these patients reactions to β-blockers that were never used before were observed. Conclusions: The diagnosis and management of patients with ACD caused by eye medications containing β-blockers can be diffi- cult: in fact patch tests used with commercial ophthalmic medications are not sensitive enough and we lack a standardized pattern of skin tests with greater diagnostic efficacy. We suggest patch testing on the classical sites (back or forearm) at a higher concentration than those contained, at present, in the ophthalmic products available on the market. If the tests are negative, it is advisable to carry out modified skin tests, such as a scratch test, to stimulate greater absorption by the palpebral skin. Moreover, it is also advisable to test all patients with a series of alternative β-blockers in order to discover possible cross-reactions, as well as testing alternative medications.

Dermatite allergica da contatto professionale da stucco di resina poliestere insatura in falegname

Viene descritto il caso di un falegname, rifinitore di infissi in legno, che usava uno stucco a base di resina poliestere ed era affetto da eczema xerotico e cheratosico delle mani. L?indagine allergodiagnostica ha dimostrato risultati discordanti in due strutture diverse. Nella prima di queste, i patch test hanno evidenziato esclusivamente una reazione positiva nei confronti del perossido di benzoile; nella seconda, oltre a questa sensibilizzazione, sono state dimostrate quelle nei confronti di balsamo del Perù, profumi mix, neomicina solfato, cobalto cloruro, anidride maleica e p-ter-butilcatecolo. Alle sensibilizzazioni riscontrate nei confronti degli ultimi tre apteni e del benzoile perossido, ovvero sostanze impiegate in sistemi a base di resine poliestere (nel caso specifico lo stucco per legno), è stata attribuita rilevanza clinica attuale. Pertanto è stata formulata la diagnosi di dermatite allergica da contatto professionale delle mani. Lo studio del caso ha suggerito agli autori considerazioni sulla clinica della dermatite palmare xerotica e cheratosica, in rapporto all?eziologia, e sulla sensibilizzazione attiva (?active sensitization? o ?flare up?), confrontando il risultato discordante degli accerta- menti eseguiti nelle due strutture.

Occupational allergic contact dermatitis to an unsatured polyester resin filler in joiner.

The case of a joiner who finished wooden fixtures using a polyester resin filler and who was affected by xerotic cheratosic eczema of the hands is reported. Patch testing performed in two different structures were discordant. Patch testing on the first structure was positive only to benzoyl peroxide, and on the second to balsam Peru, fragrance mix, neomycin sulfate, cobalt chloride, maleic anhydride and p-ter-butylcatechol. The present clinical relevance (related to the current clinical symptoms which motivated the medical advice) has been attributed to positive patch test reactions to these last three allergens and to benzoyl peroxide, all of which are used for the synthesis of polyester resins compounds (in this particular case the wood filler). Therefore occupational allergic contact dermatitis of the hands was diagnosed. The authors consider the clinical features of xerotic and cheratosic hand eczema in relation to the etiology and the occurrence of the ?active sensitization? or ?flare up?, making a compar- ison between the discordant results of the allergological investigation carried out in two different structures.

Dermatite allergica da contatto professionale disidrosiforme da guanti e scarpe antinfortunistiche in un lavoratore sensibilizzato a dietiltiourea e a tiuramici

Viene presentato il caso di un operaio edile di 21 anni, sensibilizzato a dietiltiourea e tiuramici, che ha sofferto di una dermatite allergica da contatto disidrosiforme, prima alle superfici plantari, poi a quelle palmari, con successive riacutizzazioni plantari. Si è concluso che gli episodi plantari e palmari erano dovuti ai tiuramici presenti nelle parti in gomma delle scarpe antinfortunistiche e nei guanti e che le riacutizzazioni plantari erano attribuibili anche alla sensibilizzazione nei confronti della dietiltiourea, insorta successivamente per l?uso di scarpe da ginnastica come dispositivo di protezione individuale alternativo.

Occupational dyshidrotic allergic contact dermatitis from protective gloves and shoes in a worker sensi- tized to diethyl thiourea and thiurams.

The case of a 21-year old construction worker with no previous family or personal history of atopy, who was suffering from dyshidrotic allergic contact dermatitis firstly on the soles, then on the palms with subsequent relapses on the soles, is reported. Patch testing showed positive reactions to diethyl thiourea and thiuram mix. The lesions on the soles and palms were attributed to sensitivity to thiurams present in the rubber of the protective shoes and gloves. The relapses in the soles were also due to sensitivity to diethyl thiourea which was caused by trainers subsequently worn as alternative protective footwear.

Novità nella patogenesi della dermatite allergica da contatto

La patogenesi della dermatite allergica da contatto (DAC) è stata studiata a lungo, ma riserva ancor oggi sorprese scientifiche notevoli. A questo riguardo, almeno tre nuove acquisizioni sono state rese note da recenti dati della letteratura. (1) Non soltanto le cellule di Langerhans (CL), ma anche le cellule dendritiche dermiche (CDD) sono state dimostrate essere in grado di ?caricarsi? dell?aptene e migrare ai linfonodi satelliti: in particolare, le CDD raggiungono il linfonodo ben prima delle CL, e, per di più, colonizzano aree linfonodali diverse da quelle che saranno raggiunte in seguito dalle CL. (2) In contesto linfonodale, le cellule dendritiche sono in grado di istruire i linfociti sensibilizzati verso ?l?accasamento? tissutale di destinazione: ad esempio, le CL nei linfonodi periferici programmano i linfociti T verso la destinazione cutanea. Tale accasamento, tuttavia, è stato dimostrato recentemente essere reversibile e ?plastico? : in un prossimo futuro, questa plasticità potrebbe dimostrarsi preziosa a scopi terapeutici, per quanto attiene sia la DAC che altre dermatiti T-dipendenti. (3) L?intensità della reazione di ipersensibilità da contatto si è rivelata aumentata, piuttosto che diminuita, in topi ?ingegnerizzati? privati di CL: è pertanto possibile che le CL esercitino capacità ?sottoregolatrici? , piuttosto che ?sopraregolatrici? , nei confronti della reazione medesima. Tale opportunità può essere realizzata, ad esempio, in seguito all?espressione, sulla membrana delle CL, di molecole citolitiche (quali Fas-L) in grado di esercitare citotossicità nei confronti di linfociti T specifici portatori dei rispettivi recettori (quali Fas) : tale eventualità può essere sostenuta anche sulla base della dimostrazione, ottenuta nel nostro laboratorio, che le CL attivate esprimono, al contrario delle CL ?in riposo?, la molecola Fas- L sulla loro superficie. In conclusione, l?intensità della risposta cutanea ad apteni sembra regolata dal bilancio tra meccanismi ?sopra-regolatori? e meccanismi ?sotto-regolatori? della reazione: a questo riguardo, le cellule maggiormente impegnate nello sviluppo e nel controllo della DAC sembrano essere le CL , le CDD, le cellule dendritiche linfonodali, i cheratinociti, nonché i linfociti T ?effettori? e i linfociti T ?effettori-memoria? provveduti della dotazione molecolare in grado di favorire il loro ?accasamento? nella cute.

New findings for the pathophysiology of allergic contact dermatitis.

Pathophysiologic mechanisms of aller- gic contact dermatitis (ACD) have been studied for many years: recent advances, however, are notably surprising. In this respect, at least three new findings have emerged in the current literature. (1) Although Langerhans cells (LC) were canonically thought to be capable of carrying haptens to the draining lymph-nodes, dendritic cells (DC) of the dermis were shown not only to be able to reach draining lymph-nodes well before the arrival of LC, but also to colonize lymph-node areas completely different from those colonized by LC. (2) Lymph-node DC imprint T cells for potential homing, and this homing deals with the skin in case the imprinting comes from skin-derived DC within peripheral (i.e.skin-draining) lymph-nodes. It was surprising, therefore, when it was recently demonstrated that this imprinting can be reversible and plastic. Such plasticity might be valuable for therapeutic purposes in the field of ACD as well as other T-dependent dermatoses. (3) The contact hypersensitivity reaction increased, rather than decreased, in engineered mice lacking LC. This raises the possibility that LC can exert suppressive, i.e. down- regulatory, rather than immune, i.e. supra-regulatory, capabilities. This mechanism can be shown by expressing cytolytic molecules such as Fas-L on the LC surface. Fas-L molecules are able to kill specific T cells expressing their receptors. This is supported by our demonstration that human activated, but non resting, LC, in fact, express the Fas-L molecule on the cell surface. In conclusion, the intensity of the skin immune response to haptens is as a result of the balance between supra-regulatory and down-regulatory mechanisms: such a balance is dictated by molecu- lar interactions among the different cell types resident within and/or immigrated to the sensitized skin. In this respect, the prominent cells in ACD are likely to be LC, dermal DC, lymph-node DC, as well as T effector and T effector-memory lymphocytes bearing the molecular equipment to home the haptenated skin.

Test epicutanei con farmaci: quale reale utilità?

Viene riportato uno studio effettuato in 64 pazienti con storia di reazioni avverse a farmaci, ricoverati per eseguire test di tolleranza a farmaci, a cui sono stati anche effettuati patch test con i farmaci attualmente consigliati e utilizzati da ?ESCD working party on drug skin testing in cutaneous adverse drug reactions (ESCD-WPD)?. Sono state osservate 3 reazioni positive ai patch test in 2 pazienti e precisamente positività per amoxicillina ed ampicillina in un ragazzo che aveva manifestato un?eruzione maculo-papulosa e per eritromicina in una bambina che aveva presentato orticaria diffusa. Le poche reazioni positive ai patch test registrate confermano il dato che tale metodica è dotata di buona sensibilità prevalentemente nelle eruzioni maculo-papulose ed eczematose e nell?eritema fisso da medicamenti, ma bassa sensibilità nelle reazioni orticarioidi o anafilat- tiche.

Patch test with drugs: what is their real utility?

? We report a study performed with 64 patients with an adverse drug reaction (ADR) history who were admitted to hospital for drug tolerance testing, and who were also patch tested with drugs currently suggested and used by the ?ESCD working party on drug skin testing in cutane- ous adverse drug reactions (ESCD-WPD)?. Three positive patch test reactions were observed in two patients, namely a boy presenting with a maculo-papular rash and who was found to be amoxycillin and ampicillin positive and a girl with diffuse urticaria found to be erythromycin positive. The few positive patch test reactions reported confirm that such a procedure is highly sensitive, particularly in maculo-papular rashes, eczematous reactions and fixed drug eruptions, but that it has a low sensitivity in urticarioid or anaphylactic reactions.
