Aeroallergeni e trofoallergeni: omologie e possibile ruolo patogenetico

Sono presentati i casi di 2 soggetti con storia di manifestazioni cliniche cutanee e respiratorie, risultati positivi ai test allergodiagnostici in vivo e/o in vitro per aeroallergeni vegetali e trofoallergeni vegetali. In entrambi i pazienti l?origine della sintomatologia correlabile ai pollini e di quella riconducibile al cibo erano state pressochè simultanee: ciò ci ha indotto a verificare l?esistenza di proteine notoriamente o potenzialmente responsabili di reattività crociate fra le diverse specie vegetali alle quali i soggetti erano sensibili. Allo scopo abbiamo utilizzato tecniche in silico (analisi della sequenza aminoacidica). I nostri risultati indicano come potenziali responsabili di reattività crociata la beta-espansina 1 e gli allergeni di gruppo 4. Traendo spunto dai casi clinici presentati, vengono illustrati potenzialità, limiti e prospettive della ricerca in silico quale supporto e complemento alla tradizionale e tuttora insostituibile sperimentazione in vivo e in vitro.

Aeroallergens and food allergens: homologies and their possible pathogenic role.

he cases of two subjects with a clinical history of cutaneous and respiratory manifestations are presented. One of them, a 26- year-old female, complained of intermittent asthma and several episodes of post-prandial, exercise-induced urticaria-angioedema. The other subject, a 5-year-old male child, suffered from rhinitis and frequent diffuse post-prandial pruritus. Both patients tested positive to in vivo and/or in vitro allergodiagnostic tests for some of the common aeroallergens and food allergens. Using prick tests, the woman tested positive to Compositae, Graminaceae, Dermatophagoides farinae, cereals, legumes, hazelnuts, and tomatoes, while the child tested positive to Graminaceae only. A search for specific serum IgE gave partly different results (positive for Grami- naceae and wheat in the first case, and for Graminaceae, tomatoes, wheat and beans in the second). In both subjects, avoiding the suspected foods (in the first case, avoiding physical exercise after their ingestion) led to the complete and spontaneous remission of cutaneous clinical manifestations, which was confirmed at a 6- month follow up. The simultaneous onset of pollen-related and food-related clinical manifestations led us to hypothesize and verify the existence of known and/or potential cross-reactive allergens between the vegetable species to which each of the patients was sensitized. To test this hypothesis we compared all amino acid sequences of the fully characterized proteins of the pollens of Graminaceae against those of the aforementioned vegetable foods, using the BLAST (Basic Local Alignment Search Tool) software with a cut-off value of E<0.001. Assessment of potential allergenicity was made on the basis of the FAO/WHO criteria. Our results show that the food/pollen allergen Tri a 4 of Triticum aestivum (wheat) is nearly identical to some pollen allergens (group 4 allergens) of other Graminaceae, and that the allergen beta-expansin 1 in wheat is very similar not only to other Graminaceae, expansins but also to those of Lycopersicon esculentum (tomato) and Cicer arietinum (chick pea). These data support the hypothesis that, in our patients, all clinical manifestations observed could be due to cross-reactivity caused by allergenic protein(s) which have been phylogenetically preserved in different vegetal species and suggest the need for further investigation, for both a better knowledge of allergic phenomena and possible future consequences on therapy and prevention. On the basis of the cases discussed, the paper also gives an overview on potential usefulness, limits and perspectives of in silico research as a support to, and complement of the traditional and currently unreplaceable in vitro and in vivo experimental procedures

Dermatite allergica da contatto e parestesia professionali da sigillante acrilico anaerobico in idraulico

Viene descritto il caso di un idraulico di anni 56, con anamnesi familiare e personale negativa per atopia, affetto da dermatite allergica da contatto delle mani, associata a disturbi parestesici, provocata da un sigillante anaerobico acrilico utilizzato per garantire la tenuta delle giunzioni di tubi metallici per gas ed acqua. L?esame allergodiagnostico effettuato mediante patch test ha evidenziato sensibilizzazione nei confronti dei componenti del sigillante (idrossipropilmetacrilato e idrossietilmetacrilato), del sigillante come tale (al 2% e al 5% in vaselina) e concomitanti reazioni crociate a diversi composti acrilici. Il caso viene segnalato anche per i postumi parestesici

Plumber?s occupational allergic contact dermatitis and paresthesia caused by anaerobic acrylic seal- ants.

The case of a 56-year-old plumber, with no personal or family history of atopy, suffering from allergic contact dermatitis of the hands and paresthesia of the fingers is reported. The lesions were caused by an anaerobic sealant used to secure the joints of metal water and gas pipes. Patch testing with Italian standard series (SIDAPA 2005), Acrylate adhesives series and Construction workers series showed a sensitivity to the sealant components (hydroxypropyl methacrylate and hydroxyethyl methacrylate) and to the sealant itself (2% and 5% in petrolatum). Cross-reactions to different acrylic compounds were also observed

Dermatite allergica da contatto professionale da fibre di carbonio trattate con resine epossidiche

Viene descritta una dermatite allergica da contatto professionale in un addetto alla fabbricazione di alberi per barche a vela con fibre di carbonio. Patch test effettuati con la serie standard SIDAPA 2005 e con una serie addizionale, allestita anche in base all?esame del documento di valutazione del rischio chimico e delle schede di sicurezza dei prodotti utilizzati nel ciclo lavorativo, hanno evidenziato la sensibilizzazione nei confronti della resina epossidica DGEBA (DiGlicidil Etere del Bisfenolo A), della resina epossidica cicloalifatica non-DGEBA, dell?epossiacrilato bis-GMA, dei diluenti reattivi allil-, fenil- e cresilglicidilico e della fenilendiamina ?para e ?meta. Vengono discusse la rilevanza clinica attuale professionale primaria della resina DGEBA, del bis-GMA, dei diluenti reattivi e la rilevanza clinica attuale professionale aggravante della resina non DGEBA cross-reagente e delle diammine inquinanti.

Occupational allergic contact dermatitis from carbon fibres impregnated with epoxy resin/curing agent matrix.

A case of occupational allergic contact dermatitis in a worker who makes masts with carbon fibres for sailing boats is reported. Patch testing was carried out using the Italian Standard Series (SIDAPA 2005) and an additional series specifically prepared and based on the chemical risk evaluation document and the safety papers of the products used in the manufacturing process. Sensitivity to DGEBA epoxy resin (Diglycidyl ether of bisphenol A), to non-DGEBA cycloaliphatic epoxy resin, to bis-GMA (Methacryloxyethoxy Phenylpropano-bis), to phenyl-allyl-cresyl glycidyl ethers and to p- and m-phenylenediamine was demonstrated. The present main occupational clinical relevance of DGEBA resin, of bis-GMA and of reactive thinners and the present aggravating occupational clinical relevance of cross-reacting non-DGEBA resins and of polluting diamines are discussed.

Dermatite flagellata da bleomicina in trattamento polichemioterapico

Bleomicina è un antibiotico antineoplastico isolato dallo Streptomyces verticillus che inibisce l?incorporazione della timidina nel DNA causandone frammentazione. Dopo somministrazione viene inattivata in molti tessuti da una idrolisi specifica, enzima assente nei polmoni e nella cute, sedi più frequenti di reazioni tossiche, particolarmente comuni per dosaggi elevati. Viene descritto un caso di dermatite flagellata in un paziente di 32 anni affetto da tumore a cellule germinali misto del testicolo, per il quale era stato sottoposto a polichemioterapia con cisplatino, etoposide e bleomicina

Bleomycin-induced flagellate hyperpigmentation in polychemotherapy

Bleomycin is an antibitiotic with antineoplastic properties. First isolated from Streptomyces verticillus, bleomycin inhibits the incorpora- tion of thymidine into DNA and causes splitting. Following administration, bleomycin is neutralised by hydro- lase in the tissue, but this enzyme is not present in the skin and lungs and thus high concentrations of the drug are found in these organs which are the most frequent sites of toxic reactions. The case of a 32-year-old Caucasian man who presented with a urticarial flagellate reaction after polychemotherapy with cisplatin, etoposide and bleomycin for a mixed germ cell tumor of the testis is reported

La rilevanza clinica nella dermatite allergica da contatto: proposta di scoring

Per la conferma della diagnosi di dermatite allergica da contatto sono necessarie una corretta interpretazione delle reazioni positive ai patch test e la dimostrazione della rilevanza clinica delle stesse. La valutazione della rilevanza è sempre necessaria al fine di fornire al paziente utili e non stressanti norme educazionali di prevenzione. La rilevanza, infatti, è la capacità di un sistema diagnostico-informativo, nel nostro caso i patch test, a selezionare ed evidenziare dati appropriati alle necessità del paziente. In letteratura sono pochi gli studi che trattano in maniera specifica l?argomento, e persino nei lavori clinici di eminenti esperti del settore mancano riferimenti alla rilevanza. Quand?anche considerata, i criteri di valutazione e scoring della rilevanza sono assolutamente non uniformi. A volte, tali criteri sono anche errati o discutibili. In questo lavoro si propone un inquadramento della rilevanza semplice e soprattutto basato su dati certi e incon- trovertibili, non tenendo conto quindi di dati ?dubbi?, ?probabili? o ?possibili?. La rilevanza viene suddivisa in 4 punti: presente (relativa ai sintomi clinici in atto che hanno motivato la visita), passata (relativa ad eventi clinici del passato non correlati a quelli in atto), presente e passata (relativa allo stesso evento clinico cominci- ato nel passato e perdurante nel presente con andamento cronico-recidivante) e ignota (relativa a slatentizzazione di sensibilizzazione non accompagnata da manifestazioni cliniche anamnesticamente riportate). Nel presente lavoro, inoltre, si fa riferimento ai criteri da adottare e ad una serie addizionale di esami cui ricorrere, oltre ai patch test, al fine di una valutazione il più possibile precisa della rilevanza clinica.

Clinical relevance in allergic contact dermatitis: proposal of scoring system.

In order to establish the diagnosis of allergic contact dermatitis at least two important steps should be considered: 1. the accurate recording of positive patch test reactions as true allergic reactions or false positives, and 2. the assessment of their clinical relevance. Relevance is the capability of a diagnostic system, in this case patch testing, to select and highlight data appropriate to a patient?s need. In the literature there are only a few articles that deal specifically with relevance, and, even when reported, there is no uniform concept of this definition, how it should be evaluated and scored. Sometimes, the criteria used to assess the relevance are questionable or even wrong. A relevance scoring system (4 categories) for positive allergic patch test reactions is proposed here. It refers to present relevance (related to current clinical symptoms which motivated the medical advice), past relevance (related to old clinical events, not directly correlated to the current dermatological problems), current and past relevance (related to a repeated but discontinuous harmful contact leading to a long-term clinical process), and unknown relevance (related to no known exposure and dermatitis in the past). In order to improve the assessment of the clinical relevance, a list of recommendations and a series of additional tests are also suggested

Ossido di zinco: nuovo preparato specifico per cute intollerante

Ripristinare l?integrità della barriera cutanea è l?obiettivo fondamentale del trattamento dell?eczema. Pertanto è stata valutata l?efficacia di un?emulsione acqua in olio contenente ossido di zinco alla concentrazione dell?8%. Sono stati reclutati 27 soggetti, 21 donne e 6 uomini, di età compresa tra 9 e 75 anni, affetti da dermatiti eczematose di vario tipo, dei quali 2 con anamnesi di intolleranza ai metalli. Sono stati considerati parametri clinici obiettivi (eritema, xerosi, desquamazione), soggettivi (prurito, bruciore) e, come parametro strumentale quantitativo, l?idratazione cutanea, valutata mediante determinazione della conducibilità. La tollerabilità del prodotto è stata indagata eseguendo patch test con lo stesso in 33 soggetti. Dopo 2 settimane è stato registrato un miglioramento di eritema, xerosi e desquamazione nel 64% dei soggetti, di prurito e bruciore nel 76% e dell?idratazione nel 20%. Il patch test è risultato negativo nei 33 pazienti. In conclusione l?elevata efficacia e l?ottima tollerabilità dovrebbero garantire sicurezza d?impiego, particolarmente nei soggetti con cute sensibile.

Zinc oxide: a new formulation specifically for sensitive skin.

It is well known that irritant contact dermatitis (ICD) may precede or promote allergic contact dermatitis (ACD) as damage to the cutaneous barrier allows the penetration of irritant and allergic substances through the outer layers of skin. Therefore, the most important step in the treatment of eczematous diseases, is to restore the physiological skin barrier function and zinc oxide paste has always been one of the best treatments. The present study was undertaken to assess the efficacy and tolerability of a new water in oil formulation which contains 8% zinc oxide and a very low percent- age of additives and nickel, and which has soothing and protective properties. 27 patients (21 females and 6 males), aged between 9 and 75 years (average 37) with different types of dermatitis were included in the study. All patients gave their informed consent and they were asked to apply the topical preparation twice a day for a two-week trial period. At the start (T0) and at the end of the treatment (T1), patients were assessed clinically on a 0-3 arbitrary unit (AU) score scale for objective features (erythema, dryness and desquamation) and on a 0-10 (AU) score scale for subjective features (itching and burning). Moreover, an instrumental estimate of cutaneous hydration (conductivity) was made using the Corneometer CM 800 (Courage & Khazaka). In order to test the tolerability, the product was patch tested as is on the backs of 33 healthy volunteers. All the patients completed the study and the formulation was always tolerated when applied to both affected and normal skin. Our results demonstrated a 64% improvement for erythema, dryness and desquamaton (T0: 3.33 AU; T1: 1.19 AU), 76% for itching and burning (T0: 8.07 AU; T1: 1.96 AU) and 20% for skin hydration (T0: 81.9±21.6 AU; T1: 100±19.9 AU). No positive patch test reactions or side effects were observed. The preparation has a very low percentage of preservatives and nickel and is fragrance free. It was tolerated by all and has been found to be effective in decreasing the subjective and objective symptoms of multiple dermatitis related to sensitive skin.

Reazione granulomatosa in tatuaggi: contributo clinico

Sono presentati, a scopo prettamente iconografico, i casi di 2 giovani con lesioni papulo-nodulari di consistenza duro-elastica, a margini netti, insorte nel contesto di tatuaggi, e specificamente nelle zone di iniezione di un colorante rosso. Dopo l?esecuzione dei test clinici e di laboratorio di routine, risultati nella norma, nonchè del patch test con la serie standard SIDAPA e il colorante impiegato, con esito negativo, le lesioni sono state diagnosticate, con il supporto dell?istopatologia, come reazioni granulomatose in tatuaggi. Viene inoltre presentata una panoramica sui possibili effetti indesiderati da tatuaggi, con particolare riferimento a quelli di interesse dermatologico.

Granulomatous reaction to tattoos: clinical contribution

The cases of two young male patients who came to our observation because of papulo-nodular, hard, elastic lesions, with sharp margins which appeared within their tattoos, specifically in the area where red dye had been injected, are presented. After performing routine clinical and laboratory tests which had normal results, and patch tests with the standard SIDAPA series and the aforementioned red dye which had negative results, the lesions observed were classified, with the support of a histopathological examination, as granulomatous reactions. An overview of the possible unde- sired effects of tattoos, particularly those of dermatologic interest is reported.

Sindrome orticaria-angioedema da Foeniculum vulgare

Viene descritto il caso di una paziente che riferiva saltuari e progressivamente ingravescenti episo- di post-prandiali di sindrome orticaria-angioedema. Tramite prick test con i più comuni trofoallergeni, è stata diagnosticata allergia a nocciola, arachide e orzo. Fu di conseguenza raccomandato di evitare accuratamente tali alimenti. Nonostante ciò, la paziente ha presentato recidiva della sintomatologia alcuni mesi dopo. L?esecuzione del prick by prick test ha dimostrato un?allergia a Foeniculum vulgare (finocchio), unico alimento, fra quelli assunti, non precedentemente testato. Viene sottolineata la rarità d?incidenza, le conseguenti diffi- coltà diagnostiche, nonchè la necessità di approfondire la conoscenza del profilo allergenico di Foeniculum vulgare

Urticaria-angioedema syndrome from Foeniculum vulgare: a case report

case of a 21-year old girl with sporadic post-prandial episodes of urticaria-angioedema syndrome of increasing severity is reported. Prick test with common food allergens revealed sensitivity to tree nuts, peanuts and barley. Consequently, careful avoidance of these foods was recommended. Notwithstanding, another episode occurred after some months. A prick by prick test revealed sensitivity to Foeniculum vulgare (fennel), the only ingested food which had not been previously tested. The rarity of incidence, the consequent diagnostic difficulties, and the need for further research on the allergenic profile of Foeniculum vulgare are emphasized.
