Dermatite allergica da contatto da mute in neoprene

Le mute sono costituite da neoprene, una gomma sintetica ampiamente utilizzata in relazione alle sue proprietà di elasticità e resistenza. Durante la produzione del neoprene vengono aggiunti numerosi antiossidanti ed acceleranti, tra cui le tiouree. La maggior parte delle dermatiti allergiche da contatto (DAC) da mute in neoprene è attribuibile proprio ai derivati della tiourea. Vengono presentati due casi di DAC da mute in neoprene, particolarmente interessanti in quanto non legate alle tiouree. Nel primo caso non è stata evidenziata sensibilizzazione ad allergeni della serie SIDAPA e della serie integrativa Prodotti chimici per la gomma; solo i patch test con frammenti delle mute utilizzate dal paziente hanno permesso la diagnosi. Il secondo paziente risultava allergico al nichel, verosimilmente presente nel neoprene come additivo dei coloranti. Viene inoltre eseguita una revisione della letteratura dei casi di DAC provocate da mute in neoprene.

Allergic contact dermatitis from neoprene wetsuits.

Wetsuits are mainly made of neoprene. Neoprene is a synthetic rubber widely used in different kinds of manufactures due to its properties. Several antioxidants and accelerators are added during the production of neoprene. Among these, the most com- monly used are thiourea compounds. The thiourea derivates are an uncommon cause of allergic contact dermatitis (ACD), but numerous case of ACD due to wetsuits in neoprene are attributable to thiourea compounds. We present two cases unusual of ACD related to the use of neoprene wetsuits. In the frst case patient didn?t show any reaction to allergens of the SIDAPA standard series and Rubber series. Only the patch tests with pieces of the wetsuits used by the patient allowed the diagnosis. The second patient was allergic to nickel used during the dyeing procedure.

Dermatite da contatto professionale con proteine in un panifcatore

La dermatite da contatto da proteine (DCP) è una rara patologia cutanea che interessa principalmente categorie professionali quali cuochi, veterinari, panifcatori, fruttivendoli, pescivendoli ed altri ?food handlers?, ovvero tutti quei lavoratori a stretto contatto con materiale proteico di origine alimentare. La DCP si manifesta alcuni minuti dopo il contatto con gli alimenti responsabili con reazioni dapprima orticarioidi, poi eczematose. E? descritto il caso di un panettiere atopico di 38 anni con manifestazioni orticarioidi ed eczematose recidivanti, localizzate a livello di mani, avambracci e tronco e strettamente correlate con l?attività lavorativa. I patch test effettuati su cute sana con le farine utilizzate dal paziente, opportunamente disciolte in acqua, sono risultati negativi; positivi quelli effettuati su cute lesa. I prick by prick eseguiti con le stesse farine mostravano reazione positiva immediata nei confronti di tali materiali. L?ImmunoCAP confermava tale dato ed evidenziava la presenza di Ig-E specifche nei confronti del frumento (9,76 kUA/L).

Occupational protein contact dermatitis in a baker. ?case?

Protein contact dermatitis (PCD) is a rare and underdiagnosed condition, caused by high-molecular-weight proteins, that many clinicians fail to recognize. PCD affects occupations involving food handling, such as bakers, fshmongers, cooks, greengrocers, and veterinary. Clinically, PCD is characterized by chronic and recurrent dermatitis of the hands and forearms occurring few minutes after contact with allergens. The lesions are at frst urticarial; then they develop eczematous-type, with erythema, scaling, and fssures. We report a case of an 38-year-old atopic baker who presented recurrent chronic dermatitis of hands, forearms and trunk, associated with urticaria; the lesions were related to the patient?s job. Patch tests with fours used by patient during his job were negative when performed on healthy skin, positive when performed on affected skin. Prick by prick tests with the same fours gave positive results; ImmunoCAP test showed specifc IgE antibodies for wheat (9.76 kUA/L)

Effcacia e sicurezza dell?acido ialuronico in dermocosmesi: il ?caso? Viscoderm®

I fller a base di acido ialuronico utilizzati in dermocosmesi, nonostante il continuo perfezionamento delle tecnologie di produzione e di purifcazione, continuano ad essere causa di effetti avversi. Tra quelli non immunomediati, i più comuni sono legati alle reazioni in sede d?inoculo, in parte causati da errori nella metodica di esecuzione del trattamento. Pur non esistendo differenze nella composizione chimica dell?acido ialuronico tra i vari tessuti, né tra le diverse specie, sono stati descritti casi di reazioni avverse immunomediate; tra queste, la più frequente è la comparsa di granulomi da corpo estraneo. Tuttora non è chiaro se a provocare queste reazioni siano la molecola di acido ialuronico in sé, la presenza d?impurità derivanti dai processi di produzione oppure i prodotti della degradazione della molecola. Viscoderm® è un acido ialuronico prodotto per via fermentativa da streptococco. Negli anni 2006-2011 è stato predisposto un monitoraggio fnalizzato a intercettare eventuali reazioni avverse al trattamento non riportate in scheda tecnica, condotto in 4 nazioni della Comunità europea (Italia, Gran Bretagna, Germania, Francia), sottoponendo i pazienti ad un follow up di 6 anni. Non sono stati segnalati effetti indesiderati non previsti dalla scheda tecnica.

Effcacy and safety of hyaluronic acid in dermocosmesis: the Viscoderm® ?case?

Despite a constant improvement in manufacturing and purifcation technologies, hyaluronic acid dermal fllers continue to be the cause of adverse effects. The injection site reactions are the most frequent not immune-mediated side effect. They are often related to technical mistakes during the treatment. Although there are not differences in hyaluronic acid chemical composition among various tissues or species, some cases of immune-mediated adverse reactions have been reported. The most common is the appearance of foreign body granulomas. It is still unclear if these reactions are due to the hyaluronic acid molecule itself, to the presence of impurities resulting from production processes, or to degradation products of the hyaluronic acid molecule. Viscoderm® is a hyaluronic acid produced by fermentation from streptococcus. In the years 2006-2011 it was created a monitoring system designed to detect any adverse reaction due to the treatment with Viscoderm® that was not reported in the product data sheet. It was performed in 4 european countries (Italy, Great Britain, Germany, France), submitting patients to a 6-years follow up. There have been not reported side effects not listed in the data sheet.

Interleuchine 33 e 1F9 nella patogenesi della dermatite allergica da contatto

L?interleuchina (IL)-33 e l?IL-1F9 sono membri della famiglia dell?IL-1, coinvolti nella regolazione della risposta immune innata ed adattativa. Al fne di valutare il ruolo di queste due interleuchine nella patogenesi della dermatite allergica da contatto, sono stati analizzati i loro livelli di espressione genica su cute di pazienti positivi al patch test tramite quantitative reverse transcription polymerase chain reaction (qRT-PCR). Parallelamente è stata effettuata la "ex vivo skin organ culture" sia su biopsie non lesionali di pazienti positivi ai patch test che su cute sana di controllo. I nostri dati mostrano che IL-1F9 ed IL-33 risultano espresse nella cute dei pazienti allergici, mostrando un notevole incremento dei livelli di mRNA in seguito all?esposizione all?aptene. Tali incrementi sono stati osservati anche nel modello ex vivo, che si è dimostrato un approccio effcace per lo studio di questa patologia

IL-33 and IL-1F9 in allergic contact dermatitis

Background: Allergic contact dermatitis (ACD) is a delayed type IV or cell-mediated hypersensitivity resulting from cutaneous contact with a specifc allergen. Erythema, edema, and vesiculation represent the classical clinical manifestations. Several pro-infammatory cytokines are involved in the immunopathogenesis of ACD, but little is known about the role of interleukin (IL)-1 family. These evolutionarily ancient cytokines act on innate immune cells to infuence their survival and function. In addition, they act directly on lymphocytes to reinforce adaptive immune responses. The IL-1 family members are among the most potent molecules of the innate immune system. One group consists of IL-1F1 and IL-1F2 as pro-infammatory mediators and their antagonist IL-1F3. A second functional group is formed by IL-1F6, IL-1F8 and IL-1F9 as pro-infammatory mediators and their corresponding antagonist IL-1F5. IL-33 is the most recently identifed IL-1 family member and is detected in the nucleus of epithelial cells of tissues in contact with the environment. Because of its distribution at ?border? tissues, IL-33 has been supposed to function as ?alarmin? which, similarly to IL-1α, may alert the immune system after cell and tissue damage. It may primarily act through a T helper (Th) 2 pathway but can, under certain conditions, promote Th1-type responses. IL-1F9, a pro-infammatory member of IL-1 family, is mostly expressed in keratinocytes after stimulation with Th1 cytokines. The presence of IL-1F9 in epithelial barriers suggests that this novel interleukin fulfls similar roles as their known family members (IL-1α and IL-1β), promoting response to injury or infection. Objective: To assess the role of IL-33 and IL-1F9 in ACD and to evaluate the "ex vivo skin organ culture" as a system to investigate the regulatory mechanism involved in this disease. Materials and methods: IL-33 and IL-1F9 mRNA were quantifed in lesional and non lesional skin of 12 ACD patients and in matched healthy donors through quantitative reverse transcription polymerase chain reaction (qRT-PCR). 3 mm punch biopsies from non lesional skin of 4 ACD patients and from healthy controls, who had undergone plastic surgery, were cultured and stimulated by application of the positive allergen onto the epidermis for 72 hrs. IL-33 and IL-1F9 expression levels were detected through qRT-PCR. Results: Our results show that IL-33 and IL-1F9 are similarly expressed in allergic skin when compared with non lesional skin, and their expression is even higher when compared with healthy controls. Moreover, these two interleukins are augmented in "ex vivo skin organ culture" of non lesional skin after the exposure to the allergen, while no increase was detected in normal skin, when treated with different allergens. Conclusions: IL-33 and IL-1F9 are involved in the immunopathogenesis of ACD. Allergic patients have basal higher expression of IL-33 and IL-1F9, which is increased after hapten re-exposure. Skin organ culture confrmed these results, representing a robust platform on which the immune aspects of ACD can be investigated.

L?eritromicina topica nella terapia della dermatite atopica

Staphylococcus aureus (SA) sembra giocare un ruolo importante nella patogenesi e nella espressione clinica della dermatite atopica (DA). Alcuni farmaci con attività antibatterica sono stati pertanto suggeriti per il trattamento della DA. Allo scopo di valutare l?effcacia clinica e la sicurezza di eritromicina (E) per uso topico nel trattamento della dermatite, è stato condotto uno studio randomizzato in doppio cieco ed incrociato utilizzando E 1% unguento (EU) ed il solo veicolo come placebo per 5 settimane. La valutazione clinica è stata praticata ogni 2 settimane con indice SCORAD. Contestualmente è stato valutato il grado di colonizzazione da parte di SA, sia su cute lesionale che non lesionale, nonché, mediante patch test, l?eventuale sensibilizzazione nei confronti dei topici utilizzati. Lo studio è stato completato in 19/38 (50,0%) pazienti. Questi dimostrarono un lieve ma signifcativo, miglioramento delle condizioni cliniche con l?uso di EU. Anche dopo l?utilizzo di EU, inoltre, la colonizzazione da parte di SA risultava fortemente ridotta su cute lesionale ed azzerata su quella non lesionale. In conclusione, l?impiego di E topica potrebbe essere suggerito nelle fasi più acute della malattia, in associazione con corticosteroidi topici per ridurne il dosaggio, mentre potrebbe essere preso in considerazione come monoterapia nella fase subacuta della dermatite o nel passaggio dai corticosteroidi topici agli emollienti.

Topical erythromycin in atopic dermatitis.

Staphylococcus aureus (SA) has been shown to play an important role in the pathogenesis of atopic dermatitis (AD). The density of this bacteria correlates directly with cutaneous infammation and disease severity in patient with AD. That being stated, the use of antibacterial agents should be effective in the control of this disease. Objective of this study was to evaluate effcacy and safety of erythromycin 1% ointment (EO) monotherapy in AD treatment, compared to vehicle (placebo). We performed a 5 weeks, randomized, double-blind, placebo-controlled, crossover study on 38 young patients affected with AD. Patients were randomized in 2 groups: group A and group B. All patients underwent the 1st week wash out period from topical and systemic drugs. During the 2nd and 3rd week, group A applied EO and group B applied placebo; conversely, during the 4th and 5th week, group A applied placebo and group B EO. Before the start of the study and at the end of it, SA skin colonization was investigated in lesional and non lesional areas, through skin swabs plated on to blood agar plate. Moreover tolerability and safety of study products was assessed by patch testing. Clinical evaluation was performed by SCORAD index before, during, and at the end of the study. The study was completed by 19/38 (50,0%) patients. We found that EO was able to decrease Staphylococcus aureus colonization of 64.0% in lesional skin and 100% in non lesional skin of AD patients. In addition, EO was more effective then vehicle alone in reducing SCORAD index. No contact sensitization occurred in any patient and both EO and placebo were defned safe and well tolerated. Nevertheless, although SA infection was markedly reduced, patients showed moderate improvement of clinical conditions after applying EO compared to placebo (9.2% in group A and 6.2% in group B). This demonstrate that EO alone is not effective enough to induce satisfactory improvement of clinical condition. For this reason, therapeutic use of the antibiotic is reasonable in association with topical corticosteroids, in order to reduce their application in acute phase of AD. On the other hand, EO might be considered as a possible single therapy in the subacute phase of the disease, or as a supplementary agent in the rotational therapy from topical anti-infammatory drugs to moisturizing

Moderna fsiopatologia del prurito

L?avanzamento, forse più importante, nelle conoscenze relative al prurito negli anni recenti riguarda la dimostrazione che tale sensazione viene sostanzialmente costruita, in seguito a stimoli che nascono nella cute ovvero no, a livelli centrali piuttosto che periferici: la corteccia ?localizza? infatti il prurito in sede somato-sensoriale I, mentre il grattamento è localizzabile nella corteccia pre-motoria anteriore; dal cingolo anteriore, inoltre, fbre che raggiungono le aree prefrontale e rispettivamente orbito-frontale provocano le risposte ?affettive? al prurito e al grattamento, che si traducono in sensazioni variabili dal piacere fno alla sofferenza. D?altra parte, è pur vero che la cute permane la sede classica e specifca per l?insorgenza degli stimoli pruritogeni. In tal senso, l'epidermide in toto può essere essa stessa ritenuta, in assenza dell?evidenza di recettori specifci per il prurito, un vasto unico "recettore", in grado di raccogliere i numerosi stimoli esterni e rispondere ad essi attraverso la liberazione di diversi mediatori. Questi ultimi sarebbero in grado di rappresentare, a loro volta, stimolo per recettori espressi sulla superfcie delle terminazioni libere delle fbre nervose specifche localizzate in sede epidermica e dermica; da tali recettori nervosi stimolati dipartirebbero impulsi in grado di provocare sia l?eccitazione delle fbre pruricettrici in senso direzionale centripeto, sia la sintesi e la liberazione di neuropeptidi in grado di stimolare, a loro volta, le vicine mastcellule. Queste ultime, le quali svolgono, a nostro parere, un prezioso ruolo di tipo ?cassa di risonanza?, reagirebbero a tale interessamento recettoriale liberando una serie di mediatori, il più noto dei quali è l?istamina, i quali si legherebbero a recettori specifci espressi dalle terminazioni nervose; ne deriverebbe dunque l?insorgenza di un circolo virtuoso, che può perpetuare l?infammazione cutanea sulla base della preminente attivazione di cheratinociti, mastcellule e terminazioni nervose dermiche e epidermiche, con nascita periferica conseguente di stimoli pruritogeni. Il prurito può nascere, tuttavia, non soltanto dalla descritta attivazione dei pruricettori, ma anche dalla ?sensibilizzazione? dei nocicettori periferici. Mentre, infatti, i mediatori periferici del dolore sono notoriamente in grado di attivare i neuroni capaci di trasferire gli stimoli algogeni, i medesimi e altri mediatori diverrebbero capaci, in condizioni di ?sensibilizzazione? di tali neuroni nocicettivi, di provocare stimoli pruritogeni. Peraltro, a livello centrale la ?sensibilizzazione? nervosa è in grado di evocare quadri di ?ipercnesi? e, più ancora, ?allocnesi?, che da un lato rendono più comprensibili i meccanismi patogenetici relativi a quadri clinici associati a prurito non altrimenti chiaribili, mentre riaprono, dall?altro lato, i discussi rapporti fsiopatologici tra prurito e dolore, dalla conoscenza dei quali sembrano poter derivare insperati suggerimenti terapeutici utilizzabili in condizioni pruriginose di trattamento tradizionalmente diffcoltoso. D?altra parte, il prurito può venire inibito da una serie di mec- canismi: i due più noti sono rappresentati dalla operatività dei cosiddetti cannabinoidi e, in maniera più delicata, dalla "iperattività" di una serie di mediatori, defniti vanilloidi, i quali, in condizioni "normali" di attività, stimolano invece processi pruritogeni, a partenza da recettori a tipo canale ionico, recentemente scoperti e correntemente molto studiati. In conclusione, a noi pare possibile che ogni malattia associata a prurito possegga un repertorio specifco di mediatori, recettori e vie neurobiologiche coinvolte: il riconoscimento di tali svariati repertori specifci potrà promuovere, auspicabilmente, il disegno di terapie mirate per ognuna di tali malattie associate a prurito.

The pathophysiology of itch today

Samuel Hafenreffer, 350 years ago, named pruritus as a "harmful sensation triggering a scratch desire". Today such a defnition is likely to be still useful, although a lot of studies have been performed, in the last decades, about this interesting topic. It is now clear, frst of all, that itch is built, on the basis of stimuli coming from the skin or not, at a central level. In fact, pruriceptive afferent primary fbers coming from the skin are able to bind, in the posterior horn of medulla, secondary pruriceptive neurons which project to the talamus, whereas from talamus tertiary neurons project to cortex. Cortex localizes itch in the somato-sensorial I region, while the scratch centers are localized in pre-motory anterior cortex. From anterior cingulus, moreover, fbers projecting to pre-frontal area or to orbito-frontal area trigger effective responses to itch. On the other hand, skin is still the classic and specifc area in which itching stimuli can arise. Specifcally, epidermis in toto can be regarded, in others' and our opinion, as a true large and unique "receptor", able to capture a lot of external stimuli through a variety of receptors, and consequently release a set of mediators. These mediators themselves can represent stimuli for receptors expressed on the surface of the free endings of nervous fbers localized within the epidermis and the dermis; from these stimulated nervous receptors stimuli can thus arise able to trigger both the excitation of pruriceptive fbers toward the central nervous system, and the syntesis and release of neuropeptides, which are able to stimulate mast cells. Indeed, we believe that mast cells mainly represent a sort of "resonance box", which amplifes the pruritogenic stimuli voyaging from the epidermis to the neurons. In fact, mast cell receptors favour, under the stimulation by mediators coming from both nerve endings and keratinocytes, the syntesis and the release of mast cell mediators which bind specifc receptors expressed on the nerve endings themselves: a "virtue circle" thus arises among keratinocytes, nerve endings, and mast cells, favouring the cutaneous onset of pruritogenic stimuli. Another source of itch, apart from the above mentioned "activation of pruriceptors", can be the "sensitization of nociceptors". In fact, peripheral pain mediators become able, when nociceptive peripheral neurons are "sensitized", to trigger pruritogenic, other/rather than painful, stimuli. This is, e.g., the case of "sprouting" of peripheral nervous fbers, which favours their prolonged sensitization. Moreover, a "central", other than "peripheral", sensitization exists, which opens the scenarios named "hyperknesis" and even "alloknesis": such scenarios focus the relationships between pain and itch, with consequent therapeutic opportunities, hopefully valuable for the treatment of a number of itch conditions utilizing the same drugs able to treat pain conditions. On the other hand, itch can be inhibited by a series of mediators: the two main such mediators are cannabinoids, as well as hyperactive vanilloids. Vanilloids, in fact, in normal conditions stimulate, other than inhibit, a series of pruriceptive ionic-channel-receptors, generally shared by nervous endings, keratinocytes and leukocytes. In our opinion, every itch-associated disease can have a specifc repertoire of mediators, receptors and neurobiological pathways: the recognition of these specifc repertoires may hopefully favour the assessment of a series of consequent therapeutic repertoires, which could be specifc for each itch-associated disease.

Ulcere delle gambe a tipo ectima gangrenoso da Morganella morganii, inquinante biotico dell?ambiente

Morganella morganii (Mm), bacillo anaerobio e Gram-negativo della famiglia Enterobacteriaceae, è presente nell?ambiente come inquinante del suolo e delle acque e come saprofta intestinale di umani, mammiferi e rettili. Può raramente causare infezioni della cute e dei tessuti molli e delle vie urinarie, ma anche gravi quadri setticemici. Le manifestazioni cutanee, per lo più singole se post-traumatiche e multiple in immunodepressi, si estrinsecano, oltre che con classiche lesioni ascessuali, con ulcere ectima gangrenoso-simili. E? descritto il caso di una donna di 69 anni che ha sviluppato ulcere multiple ectima gangrenoso-simili delle gambe in assenza di setticemia. L?esame istologico ha documentato ascessi dermici con granulociti neutrofli e eosinofli, stravasi ematici e vasculite necrotizzante, assenti i bacilli a livello dei vasi; gli esami microbiologici hanno consentito di isolare Mm. La terapia combinata con ceftriaxone e ciprofoxacina ha determinato rapida e persistente risoluzione del quadro clinico.

Ecthyma gangrenosum-like eruption caused by Morganella morganii, a biotic environmental pollutant.

Morganella morganii (Mm) is a Gram-negative bacillus which belongs to the Enterobacteriaceae family. It is found in the environment (water and soil) and in the bowel of humans, mammals, and reptiles as part of the normal fora. Despite its wide distribution, Mm is an uncommon cause of infections in humans. The lesions are mainly located in the skin, soft tissues, and urinary tract, but severe septicemias are possible especially in immunosuppressed subjects. Post-traumatic skin lesions are mostly seen in immunocompetent subjects, whereas multiple in immunosuppressed subjects; in this case there is often concomitant septicemia. In addition to abscess-type skin lesions, Mm can cause ecthyma gangrenosum-like eruption. An immunocompetent 69-year-old woman affected by multiple ecthyma gangrenosum-like ulcers of the legs is reported. Histopathologically there were epidermic necrosis and dermal infammatory infltrate with neutrophils, eosinophils, and histiocytes; there was no evidence of bacilli in dermal vessels. Anamnestic, clinical, haematochemical and structural fndings concurred to diagnose ecthyma gangrenosum-like eruption caused by Mm. Skin lesions quickly healed whit combined antibiotic therapy (ceftriaxone and ciprofoxacin). No relapses at 5 months follow up were observed
